Appointment Requests

This is a convenient form to allow you to request an appointment time.  I will contact you to confirm that your appointment has been booked to your satisfaction.  It is my goal to respond just as soon as I can from the time your request was submitted during regular business hours.  I work Tuesday through Saturday by appointment and I'm flexible and willing to accommodate your request, so please ask for the time that fits your schedule.  I appreciate your business and as always look forward to seeing you.
